Student verification forms coming home

September 2025

Dear Parents/Guardians:

Each year, we are required to update our student’s information. The following form(s) will be going home in the next few days and need to be completed and returned to your child’s teacher by Friday, October 3rd, 2025.

• Student Verification Form – review, make any changes & sign/date the back of form

Additional Forms that may be attached:

• Media/Website Consent Form & Internet Use form: only if we do not have from previous years or a new student to our school this year.

The Student Verification Form needs to be reviewed carefully for any errors. This is the form we refer to in the event of an emergency and it is very important that we have correct information regarding your child.

Please ensure the student’s name, address, and telephone numbers are correct.

Emergency Contacts need to be carefully checked and corrected/updated if necessary.

If there are health, medical, legal or custody issues please make note of them on the form and see the office for additional forms that may be required.

Once you have checked over all the information, please sign and date the back/bottom of the form. All unsigned forms will be returned home for a signature.

Please return all forms to your child’s teacher by Friday, October 3rd, 2025. It is important to have these forms returned in the event we have an emergency situation.
